Dr. Muhammad Ali Vajid is a board-certified Internal Medicine specialist in both the United States and Canada, with 17 years of service at Peterborough Regional Health Centre.
He has assumed responsibility for Dr. John Blakely’s Anticoagulant Clinic and continues its legacy with strict adherence to Thrombosis Canada protocols.
The clinic also accepts referrals for chronic disease management, including DM, HTN, CHF, CKD, dyslipidemia, chronic liver disease, and CAD.
When to Refer
- Patients initiating Warfarin therapy (e.g., new AFib, DVT/PE).
- Patients requiring unstable INR management.
- Peri-procedural bridging management.
- Patients needing complex dosing due to interacting medications.

Fill out the referral form (fillable PDF) and include patient demographics and indication.

We contact the patient and notify your office once seen.
What Happens After Referral
Our team assumes responsibility for INR monitoring and Warfarin dosing. We strictly adhere to safety protocols:
Initial Assessment
Review of history, medication interactions, and lifestyle factors affecting INR.
Education
Comprehensive patient counseling on diet, dosing safety, and "when to seek help".
Ongoing Communication
Reports sent to the referring physician periodically and upon discharge from the clinic.
